Notes US Library of Congress has two versions of this manuscript, a facsimile that is heavily marked up by unknown hands and was distributed for performance, and this one - Koussevitzky's original copy. This score pre-dates the piano arrangement because there are a couple pages partly glued together between which you can see an earlier version of some measures. This score remained in Koussevitzky's personal possession until his death so it can be assumed the later markings are in his hand or with his approval.
